As an authorized agent, the submitter warrants the following information and attachments as being true and correct and authorize SAGAMORE to utilize such information and make investigations, including credit investigations, concerning the undersigned, officers/principals and firm as may be deemed necessary and prudent. 
 
The undersigned also acknowledges that this application is solely informational in nature and is not to be construed as an offer by, or an agreement with, SAGAMORE, and SAGAMORE reserves the right to accept or reject any application in its sole discretion.
